**Hours**:Working two shifts per week, on a three-week rolling Rota basis 6pm - Midnight, Thursday - Tuesday.

**You must be able to work all of the shifts available and you will need to be flexible to cover the needs of the service and cover sickness, holidays etc.**

**Location**:You will be required to work at The Princess Royal Hospital community venues and service users’ homes**.**

**Salary: £35 on call shift allowance and £10.50 per hour for direct work.**

**Covid-19 Vaccination Status**: It is likely that you will need to be Covid-19 vaccinated for this role. Proof of vaccination status will be required.

**Overview of Service**:
The Mental Health Outreach and Inclusion Service (Assist and Engage) is a new initiative with an objective to provide improved mental health support for people who attend the emergency department. The project will also offer extra support and signposting for further services to those who are experiencing a delay in either transport or for a mental health admission after a mental health assessment. The service offer will differ for those experiencing delays as this may be delivered in the person’s home or other setting. It may include providing practical and emotional support to the individual and their family. As this is a pilot project, the service will develop in line with the service demands.

The outreach and inclusion support workers will offer a service to people who are medically fit to leave the Emergency Department but who may benefit from a period of additional mental health support to facilitate a safer discharge.

For people having under gone a Mental Health Act assessment (MHAA) and then experiencing delays in securing either a bed or transport, the service will provide emotional/practical support either a face to face or via telephone. All referrals for this service will be received from the Approved Mental Health Professional (AMHP) who has completed the MHAA.

The service provision will be available on call Thursday to Tuesday evenings at 6.00 pm - Midnight.

**Overview of Role**:
The outreach and inclusion support workers will be experienced mental health support workers. You will be knowledgeable and experienced in supporting people with mental health needs.

You will provide the space to listen and share, problem solve, and signpost to patients who are 18 and over, and experiencing a range of mental health problems and social stressors. This might be delivered in person or over the telephone. Examples of support to be provided include:

- Low Mood
- Anxiety
- Depress
- Self-harm
- Suicidal Ideation
- Housing

You will be required to provide solution focused support, ensuring that service users feel supported and listened to and helping them take control of their wellbeing. Individuals should be given the opportunity to explore their thoughts and feelings in a non-judgmental, confidential and safe manner.

You must be able to work on your own initiative and will need experience and qualifications relevant to the role and a desire to make a difference.

We are seeking local individuals who are able to facilitate an hours response time from point of referral. **Access to your own vehicle is essential for this role.**

For referrals from the AMHP there may be a request for a home visit to offer emotional and practical support to either the person, their family or carers.

**Main Duties**

**Development**
- Develop a service that improves accessibility and engagement with Adult Mental Health community and voluntary sector services.
- Develop and manage a robust referral pathway.
- Develop a strong and coherent knowledge of the Voluntary and Community Sector.

**Delivery**
- Provide low level mental health and wellbeing support for individuals discharged from A&E.
- Deliver a service that is flexible, person centred, and provides service users with choice.
- Provide support with problem solving and support/safety planning.
- Where necessary, signpost/actively refer to other service and agencies.
- Work collaboratively with service users towards a planned end set of goals.
- Identify suitable referrals and develop a personalised support or safety plan that reflects the individual’s needs.
- Deliver a service that is flexible, person centred, focuses on people’s strengths and personal goals and provides them with choice.
- Co-produce a personalised support plan with the service user to improve their mental health and wellbeing, introducing or reconnecting people to community groups and statutory services as appropriate.
- Deliver a range of direct interventions depending on individual needs including;
- Emotional support including support to develop effective coping strategies (working therapeutically)
- Support to regain or maintain support networks
- Support to develop more confidence and a greater abilit
