**Your role and responsibilities**

You will:

- provide professional support to practitioners within the team alongside wider management support to include the delivery of integrated care and health services
- be responsible for the delivery of the councils strategic and visionary agenda to provide effective and best value services for the people of Suffolk
- provide senior support and oversight as part of the team alongside a degree of management cover to support the team manager
- have good knowledge of the Care Act 2014 and Mental Capacity Act 2005
- be responsible for providing effective senior oversight through promoting a Signs of Safety approach to supervision and case management which may include legal intervention and complexity
- hold a small caseload within this role which will involve being responsible for managing complex and challenging cases, alongside supporting practitioners with case management and good practice
- need to support the Team Manager in the leadership and management of the team, ensuring effective operations in planning, programme development, human resources and staff development, and financial oversight.

**You will need**:

- to promote a high-performance culture within the team that is innovative, responsive to changing priorities, flexible and empowers people to deliver excellence, best value and continuous improvement for the people of Suffolk
- a clear focus on customer, families, carer and community needs, in the context of demand reduction, ensuring preventative and long-term sustainable solutions which enable people to live long and fulfilling lives by maximising their independence
- to support the Team Manager in the Leadership and management of the area teams, ensuring effective operations in planning, programme development, human resources and staff development, and financial oversight.

**The team**

You will form part of the Integrated Neighbourhood Teams and provide support and advice to frontline staff delivering social work services, occupational therapy, reablement services, community nursing and physiotherapy.

You will be expected to hold a small caseload to ensure that you maintain your professional focus. You will work with local Contract and Service Development Teams and managers to understand, develop, and share good practice, ensuring frontline teams share information to inform future commissioning models and new ways of working.
