An exciting opportunity has arisen for a care coordinator (admin role) to work directly with the Neighbourhood team. The post holder will be responsible for the provision of wide-ranging and efficient administrative support to the Neighbourhood Team.

First Coastal PCN covers a population of approximately 56,000 this is made up of three practices, Marsico Medical Practice (Mablethorpe), Hawthorn Medical Practice (Skegness) and Beacon Medical Practice (Skegness). These practices work together across the geographical area of Mablethorpe and Skegness to deliver contracts and services for their GP registered population.

Lincolnshire Community Health Services NHS Trust (LCHS) is the primary community healthcare provider in Lincolnshire, rated Outstanding by the CQC. The Trust delivers community-based services aimed at supporting people to manage their own health at home and reducing the need for people to go into the hospital.

The care coordinator will have overall responsibility for the coordination of the regular Multi Disciplinary Team meetings and the tracking of patients who are on the caseload of the Neighbourhood Team. This role includes completing all administrative, information and organisational duties to ensure maximal outcomes from meetings and coordinated care for patients.
The key role of the coordinator will be to schedule all meetings across all the general practices in their cluster.
