Lincolnshire's Community Childrens Nursing Service is expanding

We have a rare and super exciting job opening for a compassionate, inclusive Team Leader with a passion for community children's nursing to join us and set up our 4th CCN Team in the County

You will be instrumental in developing a local CCN service for the children, young people and families residing within the East Lindsey District.

You will be responsible for the operational management and leadership of the team Monday - Friday 9.00am to 5.00pm providing direct nursing care, support and education to children and young people with acute nursing needs, long-term, complex medical and life-limiting conditions and their families.

Working flexibly, you will meet the needs of individual children and families offering 'before' and 'after' school visits as required.

You will participate in a On-Call Telephone service to the county-wide CCN caseload Monday to Friday 5.00pm -10.00pm and at weekends 9.00am -10.00pm including participating in the 24/7 On-Call rota for children requiring end of life care.

Applicants must be NMC Registered Nurse (Child) with experience as a Band 6 Community Childrens Nurse or experience leading a children's community team.

An MSc / BSc (Hons) in Community Health with the Childrens Specialist Practitioner qualification or equivalent experience is essential. Qualifications in Leadership or equivalent experience and Teaching and Mentoring are also essential for this role.

You will have responsibility to lead your team and provide high quality safe nursing care to children, young people 0-19 years and their families at home and in various community settings.

Working collaboratively across multi-agency / organisational boundaries you will initiate and coordinate appropriate services to facilitate the early discharge, prevent hospital admissions and manage children and young people with long-term conditions and complex medical needs at home. You will have the confidence and ability to liaise with professionals within primary care and community health services, social care, education and voluntary services.

You will provide direct nursing care and have responsibility of managing your own caseload. In partnership with individual children and young people and their families and all professionals involved in their care, you will formulate personalised holistic packages of care which incorporate the child / young persons feeling, wishes and goals (where possible) and promote the ethos of empowering children and their families to confidently self-care.

**This will include preparing them for healthcare transition**: working closely with the GP, District Nurses / Specialists, Consultants, Specialist Tertiary Centre and our Roald Dahl Transition Nurse Specialist to ensure young people have a positive experience in their journey across to adult services.

This new team will be part of our well-established CCN Teams and form part of our wider Children's Community Services.
