Job description

We are looking for Mental Health Social Workers and have positions available in the following areas:

- The Centralised Social Work service aligned to our 3 MINT Locality teams.
- Rapid Discharge Hospital Team
- Reviews Team

The Ealing Adult Mental Health Social Care Service comprising of, the Centralised Social Work service, the Approved Mental Health Service, Reviews Team and Rapid Discharge Hospital Team and the Substance Misuse team.

We work in a multi-disciplinary way with West London NHS Trust and partnering agencies. To deliver tailored support plans that focus on recovery, preventative work, and effective risk management. The team is aligned with three Mental Health Integrated Network (MINT) teams, located in the East, South and North of the Borough.

The Rapid Discharge Hospital Team: Ensures a smooth and effective discharge pathway from hospital to the community. This is a small, resolute team that conducts statutory assessments, collaboratively with service users, their families, and multidisciplinary teams. They work closely with NHS colleagues from acute inpatient and general hospitals and rehabilitations facilities and with community mental health teams to facilitate safe, timely discharges. Setting up packages of care and support planning, utilise reablement and other services.

The Reviews Team: Undertake annual and unscheduled reviews for those receiving commissioned care and support services and engage in Safeguarding adults’ activities. This team has a great focus on promotion of independence and works closely with providers and the Community Rehab Team to support prole into lesser restrictive environments.

**What are we looking for?**

Conscientious Social Workers with professional curiosity and robust assessment skills. The post holder (s) must have demonstrable post qualification experience, ideally in mental health services.

Have a good working knowledge of the relevant legal framework / legislations, including the Mental Health Act 1983 amended (2007), Human Rights Act (1998), Mental Capacity Act (2005) and Care Act (2014) and government guidance on Adult Safeguarding.

You will have excellent verbal and written communication skills, be able to engage effectively with people at all levels, active listening skills. Desirably you may have relevant post -registration qualifications such as, BIA, AMHP, Practice Educator.

We will require you to have a minimum of 2 years post-qualification experience for grade 9 and 10.
