Adults Social Worker - Community Mental Health Team

Reed is working with Barnet Council to recruit an experienced Adults Social Worker to join its Community Mental Health Team (CMHT). This is a fantastic opportunity to work with adults who are diagnosed with mental health conditions, including dementia, psychosis, anxiety, and depression.

Key Responsibilities:

  • Conduct holistic needs-based assessments and develop care plans to support adults with mental health conditions.
  • Work collaboratively with health and AHP colleagues to provide comprehensive care and support.
  • Implement the Care Act and Mental Capacity Act, ensuring that adults receive the necessary support and care.
  • Manage risk and develop strategies to mitigate potential risks.
  • Work within the Care Program Approach, ensuring that adults receive person-centered care.

Requirements:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have a Social Work qualification (Diploma in social work, social work degree, or equivalent).
