Overview:
We are seeking a highly organised and compassionate individual to join our team as a Care Coordinator. As a Care Coordinator, you will play a crucial role in assisting clients and their families in navigating the care system and coordinating their care. This is an office-based position that requires excellent communication and organisational skills.

**Duties**:

- Assist clients in understanding their care options and making informed decisions
- Collaborate with our care team professionals to ensure continuity of care
- Maintain accurate and up-to-date client records
- Provide support and guidance to clients and their families throughout the care process
- Assist in the development and implementation of care plans
- Monitor client progress and make necessary adjustments to care plans as needed

Qualifications:

- Previous experience in a healthcare or social services setting is highly desirable
- Strong organisational skills with the ability to prioritise tasks effectively
- Excellent verbal and written communication skills
- Proficient in Microsoft Office suite (Word, Excel, Outlook)
- Ability to work independently and as part of a team
- Compassionate and empathetic nature
- Attention to detail and strong problem-solving skills

Please be aware, we take a carer-centric approach to everything we do. We prioritise our care team so that they can prioritise our clients. As our care co-ordinator you will be a main point of contact for our team, and have to empathise with and appreciate that they have a difficiult job to do.

**Salary**: £21,881.70-£24,900.52 per year
