This is an exciting new opportunity to be part of an innovative new service optimising resources within Chesterfield/North East Derbyshire, reducing demand and releasing capacity and more importantly empowering patients to self-care and promote their independence through behavioural change models, using a goal setting approach to achieve this.

The Self-Management facilitators team currently provide a programme of interventions in Derbyshire, and are now expanding into Chesterfield/North East Derbyshire and looking to recruit new members for the SMF team.

The overall aim of the service is to work with individuals to increase their knowledge, skills and confidence to manage their long-term conditions and care needs.

You will continue to promote patient centred care and utilise the shared decision making tools.

This job allows for innovative thinking and collaboration with other providers to support individuals with a sustainable change that means the most to them.

This is a clinical role and will involve care delivery with a behavioural change focus.

**Examples of care delivery that may be delivered are**:
Insulin administration and other injections

Catheter care support

Simple wound management

Medication administration

With the scope and training to continue to develop clinical skills and behavioural techniques.

We deliver support patients in their own homes and in the local acute hospitals, working with individuals at the point of discharge so there will be lots of variety whilst working in this developing role.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response, we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

Providing patient care in their own homes.

Duties may include catheterisation and catheter care, simple wound care, compression hosiery, injections, insulin administration and diabetes support.

The applicant will be expected to travel across the county in their own vehicle.
