**ROLE RESPONSIBILITIES AND TASKS**:

- **Main Duties using a holistic person-centred approach**:_

1. Set up activity programmes for home and individuals’, short
- and longer-term planning for each resident, including 1:1 and group sessions, seeking out equal opportunities for all.

2. Planning and involvement in facilitating theme days, wish tree, summer fayre and seasonal events.

3. To plan and support in assisting residents in aspects of independent daily living i.e. laundry, cooking and cleaning skills to develop these further where possible to promote independence.

4. Purchase activity equipment as required and seek visiting professionals to meet the service needs and requests of residents.

5. Facilitate activities and direction of delivery of activities as per planned programmes and adapt these as necessary ensuring they meet specified 1:1 funded hours. Auditing of these planners and 1:1 hours being met and evidenced.

6. Monthly residents’ meetings, reflecting and acting on requests, working collaboratively with other staff and management.

7. Liaise with the local Community to seek out opportunities to reintegrate with community living and promote rehabilitation skills and socialisation.

8. Provide supervision for volunteers, junior staff and work experience students that may be working in the home or out on activities.

9. Involvement in fundraising for the home.

10. Use physical handling techniques and hoists safely as per Moving and Handling Training.

11. Support communication needs according to comprehension level of the resident, including use of augmentative communication strategies.

12. Be proactive in involvement with resident families and friends as appropriate to arrange events and home visits.

13. Further develop and maintain residents’ mobility and movement skills, incorporating knowledge of impaired neurological function on movement and muscle tone, motor and sensory development, and its effect on behaviour and cognitive abilities.

14. Assist the staff when requested with the day to day care of residents in the home or whilst supporting in the community.

15. Ensure all information is logged or effectively documented using current methods of the home. Evidence can be provided to professionals upon request for proof of engagement or refusals to evidence care or plans being delivered. Pass on information that may be relevant in handover books, care plans and daily logs.

16. Provide supervision for volunteers, junior staff and work experience students that may be working in the home or out on activities.

17. Attend staff meetings, group focus group session, team meetings and training wherever possible.

18. To undertake any other duties which are consistent with the post or as directed by the Management staff

**Legislation and Health and Safety**:

- Be responsible for raising the Emergency alarms as demonstrated in Induction Training for fire and or Nurse call systems.
- Use physical handling techniques and equipment safely as per Moving and Handling Training.
- Keep the home clean and tidy during any activity sessions and keep fire access to doorways clear at all times.
- Undertake and produce appropriate risk assessments ensuring all new activities are risk assessed for safety, these are then to be checked by management.
- Ensure that all therapy equipment, materials and tools are clean and safe and fit for purpose.

**- Adhere to all Health and Safety & Infection Control requirements of the service.**

**The residents and their family and or representatives**:

- Be proactive in working with residents, and their families.
- Have a basic knowledge of the impact of disability on the adult and their family and the restrictions this may impose.
- Be aware of safeguarding issues and act promptly regarding any concern in accordance with guidelines.
- Be aware of when and families may need to access other services both internally and externally and discuss with therapists.

**Continuing Development of Self and Others**:

- Be responsible for and record your own continued personal development (CPD).
- Attend and actively participate in regular supervision, appraisals and staff meetings.
- Attend external and internal courses, feeding back new knowledge through in-house training.
- Demonstrate increasing knowledge through improving and advancing own practice and contribute to the promotion of practice across the service.

Actively contribute to the service outcomes and policies.

**Salary**: £11.00 per hour

Expected hours: 30 per week
