**Responsible for**: Community Carer

**Job Summary**:
Care Co-ordinator need to be first line managers, supervision and rota responsibilities as well as on call duties.

The role will promote and ensure the company’s ethos in person centred support whilst maximising an individual’s potential and independence.

The role requires a high level of organisation and planning skills, it is vital that this person can adapt their communication styles to meet the needs of individuals, lead by example and to demonstrate safe effective practices. This position will expect you to be a key member of the team in working to improve and develop service quality and delivery. You will take an active part in mentoring and developing individual members of staff and strengthening a team around the people we support.

It also requires a degree of autonomy, initiative and responsibility from the post holder whilst ensuring compliance with company legislation, policies and procedures are adhered to at all times.

It will therefore be necessary for the Care Co-ordinator to respect and work co-operatively with others enabling the people we support to live in their own home or within their local community with dignity and confidence.

The Care Co-ordinator role is essential to the wellbeing of the people we support and to the development of the team of people employed to support them.

**Duties**:

- Service Requirements_
- Adhere to policies and procedures.
- Adhere to legal standards and maintain levels within them
- Contribute and be aware of personal responsibilities and accountability in ensuring a safe working environment is maintained in accordance with health and safety legislation, fire safety legislation and lone working policies
- Report any defects to property or equipment as soon as possible and follow up any actions
- Provide quality monitoring reports when required to the Registered Manager
- Be involved in fact finding and information gathering where complaints or concerns have been raised
- Organise the facilitation of new client contracts
- To the people we support_
- Ensure the support provided reflects the wishes of the individuals we support and guide other team members through these
- Ensure all the person’s specific support plans, protocols and risk assessments are communicated, adhered to and reviewed at the correct times
- Ensure and monitor the safe ordering, dispensing and storing of medications and accurate recording of the administration of medications in accordance with the medication procedure and the person’s medication protocol
- Ensure and monitor support staff to encourage a person to maintain and develop a high standard of social, self-help, occupational and personal skills to enable them to be as independent as possible within and outside their home environment
- Devise and monitor with an individual their health action plan and facilitate and encourage them in maintaining their health and wellbeing and to seek appropriate advice with a person when necessary
- Where appropriate support and advise people with finances and budgeting and where a recording system is in place monitor this
- Ensure and monitor that a person is empowered at every reasonable opportunity to make decisions and take control over their life and service within the structure of the mental capacity act
- Ensure and monitor that all staff work in line with the core principles and service policies around the safeguarding of vulnerable adults
- Ensuring that people are able to move their lives forward looking for community connections, friends, hobbies, interests, work and being creative and challenging about the support provided
- Be a point of contact to family, friends and professionals in relation to an individual
- Be creative in involving the people we support in service or individual meetings, which ensure each of their wishes / opinions are valued
- Work with the service user in planning their support time or 1-1 time and communicating activities to the rest of the person’s team
- Staffing/Personnel Responsibilities_
- Carry out duties in line with instructions given by Registered Manager
- Provide supervision, induction and mentoring to staff within your team and offer direction, guidance and advice on everyday matters around an individual
- Provide support and direction to trainee’s on placement i.e. students, apprentices, volunteers or people on work experience
- Support staff to work within boundaries of their role and capabilities
- Actively contribute to a staff member’s personal development plan and highlight training needs where appropriate to your Registered Manager
