The post holder will contribute to the planning, implementation and evaluation of a range of health improvement programmes that promote health and well being, protect health, encourage healthy lifestyles and reduce health inequalities in line with GMMH agreed needs and priorities
- The post holder will provide specialist advice and support on the development of health improvement programmes.
- The post holder will be responsible for establishing and sustaining links with key external partners and a range of stakeholders, engaging them to support the coordination, promotion and delivery of interventions that support the agreed areas of work, and liaising with strategic forums and/or national organisations in order to deliver key objectives
- The post holder will promote health services within the establishment by developing promotional activities to raise awareness of the services being offered and take the lead in developing new services.

1. To be responsible for the implementation of key areas of Health Improvement and inequalities work within the public health agreed annual health improvement plan.
2. In collaboration with public health colleagues, take a lead role in a range of settings in the facilitation and development of projects and programmes to promote health and well-being and reduce health inequalities within the prison community.
3. To raise awareness of health improvement issues amongst target populations and increase access and utilisation of primary and secondary prevention services by providing health improvement advice and information which supports the care and education of local communities.
4. To play a role in the planning, securing of funding and initial implementation of projects and programmes to promote the health and well-being of the community within HMP Risley & Thorncross
5. To develop and maintain partnership and networking opportunities across the community, statutory and voluntary organisations, focusing on raising awareness of issues relating to specialist topics and wider determinants of health.
6. To attend staff and wider Health & prison meetings.
7. The post holder will liaise effectively with the local community and other Agencies/ Partners and build and sustain robust relationships / networks.

1. To work closely with key partners who can influence the Health Improvement and Inequalities programme across the partnership, e.g. Health care, Local Authority, CSU, Acute Trust, and Voluntary sector staff.
2. To identify the training needs of healthcare and other professionals and facilitate the delivery of programmes that will increase their skills and capacity to enable them to improve health and reduce inequalities in health.
3. To use project management tools to ensure all health improvement interventions or events have clear aims and objectives, are monitored and evaluated, sharing best practice as appropriate
4. To develop community capacity through the provision of training and skills development and to enable community members to make decisions and be able to develop community-led health and well-being projects
5. To analyse health data and related health information to identify local health needs in order to inform policy and practice in a targeted way.
6. To produce accurate reports, monitor and evaluate projects as required, ensuring work conforms to and meets the standards and targets set by the Public Health Outcomes Framework.
7. To undertake appropriate professional development as identified in the post holder’s personal development plan.
