Young Carers Practitioner

We are recruiting for a Young Carers Practitioner to join our Warrington Carers service, helping young people who have caring responsibilities.
This is a varied and rewarding role which benefits from true Hybrid Working, involving a mix between community working with young people, office working with a great team and home working for added flexibility and work-life balance.
To support our young carers, a Young Carers Practitioner would be expected to carry out the following tasks:
- Contact new Young Carers and welcome them to our service
- Gain an understanding of their caring role and develop individual support plans that reduce the impact of their caring responsibilities
- Make appropriate referrals for a statutory Young Carers Assessment
- Provide ongoing, time bound, one-to-one support, regularly reviewing their needs and adapting support plans as required
- Build relationships with and refer Young Carers and their families to broad range of agencies and community assets which meet their needs
- Support Young Carers to understand the condition of the person they care for and provide information on health and social care services
- Participate in multi-agency meetings such as team around the family, child protection conferences and child-in-need reviews
- Develop and deliver community-based peer support opportunities, such as peer support groups, activities and trips, including evening, weekend and out of term time opportunities
- Develop and deliver training opportunities for Young Carers to help them in their caring role and to provide skills for the future
- Record and maintain accurate and confidential records
- Use technology and social media to provide information, advice, and support to young carers
- Help identify Young Carers through a range of community and engagement events
- Promote and safeguard the welfare of children and young people and adults
The successful applicant will demonstrate capability to:
- Work with children and young people, individually and in group settings
- Use Microsoft Office packages
- Understand and carry out safeguarding and child protection procedures
- Complete assessments and support plans
- Motivate and encourage others
- Manage a complex and varied workload
- Work well as a team and individually
- Commit to anti-discriminatory practice
- Commit to the values and aims of n-compass
**Successful applicants will be required to undergo an enhanced DBS check
