Operations & Quality Co-ordinator

**Working with the Operations team to**: Integrate all new clients into Crossroads processes and procedures following the Care Assessors 6-week review and handover. Ensure that the care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards, by an effective and accurate management of the weekly rota, recording absences on WebRoster and liaising with HR on the management of such absences, and by reporting staff non-compliance to the Care Manager/Registered Manager Undertake half-yearly reviews (earlier if required due to reported changes of care needs) of care for all service users, including risk and Mental Capacity Act assessments, updating all records and client/service user files accordingly. Address any concerns service users or field staff have raised about the quality of care, escalating issues to the Care Manager when and where appropriate. Mentor all new staff for their first six months of employment, by attending the final shadow shift to ensure competence before formal sign off, conducting probationary reviews and continuing to mentor and support them throughout the remainder of their probationary period and onwards as required, feeding back to the Care Manager any areas of concern so that action plans can be put in place to address performance, attitude or behaviour.
Support new staff in their role and in the completion of their Care Certificate and other training as appropriate. Provide support and mentoring for other staff, where care delivery concerns have been raised or if further support is needed. Conduct regular audits, spot checks and observations to ensure only the highest quality of service, identifying any gaps in training for all care support staff, and recommend solutions to these gaps. Conduct risk assessments for pregnant care staff at 12 and 30 weeks, and between times if necessary, supporting staff through their pregnancy and on their return from maternity leave, and for other care staff where necessary, following from or to prevent an absence from work.
Undertake all other relevant risk assessment required in line with CQC and H&S regulations. To be innovative and creative in looking at ways to improve service delivery. Proactively ensure that all systems and documentation across the service are monitored and maintained in a timely and accurate manner and that any changes are communicated to all relevant teams. To liaise with Social Workers, GPs, Hospitals, Occupational Therapists, Pharmacies and family members/advocates in relation to the Service Users ongoing needs and actions required and report on any recommendations.
To promote the independence of service users. Undertake direct provision of care as required, but particularly on short notice (e.g. cover for a colleague who has called in sick) or in emergency situations (e.g., death of a Service User, taking over the rota from Support Worker). To participate in the on-call service on a rota basis.
To ensure that you have a firm and up-to-date knowledge and understanding of current legislation, regulations and policies underpinning the delivery of care in the health care sector. To share the responsibility of ensuring the safe delivery of the services within the community (e.g. reporting findings, acting on concerns and overseeing the improvement provisions in the aid of the delivery of service). To undertake any other duties assigned by the Care Manager from time to time.
Such duties will be reasonable in relation to the post holders skills, abilities, and status.
