Bridging Care and Rehabilitation Service Team Lead

The post holder will work in partnership with NELFT community services, Waltham Forest Social Care and other agencies to provide short term, responsive and quality homebased care and rehabilitation for service users.

The post holder will be responsible for the operational management of The Bridging Care and Rehabilitation Service and to ensure care is delivered in line with agreed care plans. This will include staff management and rota coordination to ensure safe delivery of the service 7 days a week between 0700 to 2100 hours

The service offers short-term homebased care and rehabilitation with the aim to:
Facilitate timely safe discharge from acute hospital care

Help prevent unnecessary admissions to care homes

Help prevent unnecessary hospital admissions

Help to regain confidence with living skills or support with long term care needs until such time a Social Services / private package of care can commence.

Promote a reablement and rehabilitation approach to care to enable patient to regain a level of independence.

The post holder will supervise Support Workers and Coordinators to ensure a safe service is maintained. The post holder must be practical, solution focussed and able to work on their own initiative. The post holder must be highly organised and be able to adapt to continually changing situation.

You will demonstrate a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies

To lead, support, motivate and develop a team of Support Workers and provide day to day operational management of The Bridging Care and Rehabilitation Service.

To develop rotas to ensure the service is staffed safely, including booking of temporary staffing, where indicated.

To maximise resources through the efficient deployment of staff within the borough.

To plan and organise Bridging Care and Rehabilitation Service daily rosters, coordinating shift to ensure all visits are completed and staff safety is maintained.

To have oversight of relevant developments for each service user following feedback from support workers and adapt care plans accordingly

To participate in a duty rota, as required to provide care and rehabilitation to service users to include unsociable hours.

To have knowledge relating to Trust protocols and procedures and adhere to them, particularly manual handling.

To be responsible for providing accurate records of information required by the Trust for audit purposes. To be compliant with all aspects of RiO, including agreed operational processes for its use.

To maintain statistical returns as required by the Organisation, including entering data onto the RIO system.

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

To ensure that safeguarding is a key priority and that all safeguarding concerns are addressed in a timely manner.

To review service users’ progress regularly and ensure that any proposed changes are agreed with the service user and/or their family/carers.

To be skilled in communication, establishing and maintaining good relationship with service users, carers, members of the wider health and social care team or other agencies. Reflect on practice regularly and plan personal development in order to achieve growth development. Access supervision from the Integrated Hub Lead.

To carry out any other duties as and when requested by the Management Team.

To maintain confidentiality in all dealings with service users and their records.

To support service users, relatives and other care imparting knowledge to promote positive health and safe use of available equipment.

To have knowledge of and be able to effectively use available resources.
