Main duties and responsibilities To adhere to the Care Certificate standards and all other relevant guidance from the Trust. To be aware of the Health and Care Professions Councils (HCPC) codes of conduct. To establish and maintain effective working relationships both within the Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values. To support registered dietitians and speech & language therapists to assess patients.

To contribute to the development and implementation of programmes of treatment/ care/support for individuals, families and carers under the guidance of a registered clinician. The care/support plans may include the following: Promoting independence/self-management and maintaining safety Increased functional ability Improvement in health and wellbeing including mental health Interventions in disease/condition management Overcome barriers to understanding, e.g. patients with hearing or cognitive impairment Prevention and reducing of health inequalities Exchanging factual information, reassurance, tact and empathy. Admission avoidance Early supported discharge Contribute to case management/ treatment /care plans Palliative and end of life care Specialised interventions relating to dietetics.

Specialised interventions relating to speech & language therapy in terms of communication and swallowing difficulties Demonstrating dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking weight, Mid Upper Arm Circumference (MUAC), etc Protection of Children & Vulnerable Adults To promote and safeguard the welfare of children, young people and vulnerable adults. To maintain and develop skills and knowledge to deliver a range of interventions and set an example of good practice to other team members including skill sharing. To maintain contemporaneous, timely and accurate records as required by the Trust and in line with standards.

Support identified team members giving guidance and direction where appropriate To take responsibility for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately and for transportation of equipment between locations. To work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc. Under the in-direct supervision of a registered dietitian and speech & language therapist; provides advice to patients, carers and professionals regarding the management and care/support of patients Work with qualified dietitians and speech & language therapists with group work/clinics as required. To use discretion to prioritise delegated work from a registered clinician.

To be involved in audit and research in accordance with the needs of the service. Participate in student training placements by being shadowed by student dietitians and SLT Be competent as appropriate to the requirements of the role. Work autonomously at a level appropriate to clinical competence, planning and prioritising own visits to delegated patients. Be skilled in supporting the development, implementation and evaluation of care/support packages under the direction of a registered clinician.

Demonstrate an awareness and understanding of consent, and gain consent as appropriate following organisational policies. To attend relevant external and internal courses to extend knowledge or gain relevant skills to improve clinical practice as identified in the Personal Development Review (PDR). To attend staff meetings and any other relevant meetings as required To participate in regular supervision in accordance with good practice guidelines and Trust policy To participate in the Trusts annual Appraisal process To complete all relevant mandatory training as and when required to do so Systems and equipment Daily use of clinical equipment such as weighing scales, tape measures and training materials. Ensure all equipment is checked and maintained ready for use according to the manufacturers instruction.

Daily use of IT systems such as Rio, i-Portal, Service to Service and Microsoft office packages To comply fully with the duties and responsibilities outlined in the Trusts Information Governance Policy To comply with the Data Protection Act 1998, NHS Confidentiality guidelines (e.g. Caldicott, GMC.) and any code of practice on Confidentiality and Data Protection, as accepted by the Trust. Departmental codes of practice and procedures for confidentiality are available from your operational lead To ensure that all information collected, stored and used is done so in compliance with the above Act and any relevant Trust Policy To preserve the confidentiality of any information regarding patients, staff records in your area (in connection with their employment) and the Trust business. This obligation shall continue in perpetuity.
