**Main Duties & Responsibilities**: To engage with people who have substance misuse problems and providing them with advice, screening and clinical interventions to address their substance misuse issues, and primary health care. To carry out comprehensive assessments and contribute to the development of individual recovery plans with those who express a wish to access community treatment/support services. As a nurse prescriber, to ensure that service users are fully informed and have given consent to treatment. To titrate patients safely on to a treatment regime (where required) and ensure that procedures are constantly reviewed to promote best evidenced based practice in accordance with clinical guidelines and local procedures Ensure that service users who are dependent on alcohol are assessed for their suitability to access a community detoxification.

Deliver community-based medical detoxes in conjunction with an appropriate package of care, where it has been assessed as safe to do so. Deliver a range of clinical interventions, including HCV /HBV testing and vaccination and coordinating the onward referral of those diagnosed with a blood borne virus. Raising awareness of blood borne viruses, their transmission, treatment and the significance of infection Undertake phlebotomy and dry blood spot testing, as required, to undertake routine screening and testing, ensuring adherence to identified pathway for blood sample labelling, collection, obtaining results and signposting follow-up and any next steps planning. To offer wound care assessment and basic wound dressing to clients.

To refer and liaise with other agencies involved with the clients broader care plan as part of the Service, including Health Services, hospitals and GPs. Make referrals to local specialist teams for an inpatient detoxification, (when the service users medical history or personal circumstances are not suitable for community detoxification). To develop and progress community detoxification, taking the lead in ensuring that the service has a comprehensive and open response community detoxification. Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and clinical protocols and procedures.

Participate in clinical audits, service reviews and the evaluation of service user satisfaction, implementing resulting recommendations, in order to improve and develop the service To maintain an up to date knowledge of medicine management and both local and national guidelines and agreements. To attend meetings appropriate to the development and maintenance of the role To adhere to Patient Group Directives and ensure requirements on updating are maintained To receive regular clinical supervision and maintain a current knowledge of appropriate interventions Emotional Effort Occasional exposure to highly distressing or highly emotional circumstances. Occasional exposure to aggressive patients. Working closely in clinical practice with patients with substance use.

The backgrounds and presentation of many of these patients can present exceptional challenges in working with the patients, in managing personal emotional responses and those of others e.g. carers Criteria Essential Desirable Qualifications First level registered nurse with a current PIN. A V300 Qualification for independent/supplementary prescribing with current NMC registration Sound and demonstrable knowledge of clinical prescribing issues including alcohol detoxification Knowledge of the issues facing substance misusers including appropriate medical interventions, social care and health issues Specialist knowledge and demonstrable competencies in relation to substance use, dependency, and detoxification Qualification(s) in substance misuse Recent relevant post qualification experience A clear understanding of the need for and ability to deliver quality services RCGP Qualifications Level 1 (and 2 or willingness to work towards) Training Ability to assess, plan, implement and evaluate patient care Knowledge of the safe storage and administration of medication requirements Trained in phlebotomy Injection administration Awareness of health issues related to substance use
