To work as an Advanced Practitioner in the field of post-COVID-19 syndrome. To contribute to the effective working of the long-term health condition management teams. Leadership and Management Pathway Development The post holder will lead the development of clinically appropriate pathways for people with post-COVID-19 syndrome to ensure that they are able to access appropriate services to support their recovery. Contributing to Transformation Programmes The post holder will take an active role in contributing to the Health Boards Transformation Programmes as agreed with the Head of Service.

Service Improvement Projects The post holder will lead and contribute to service improvement projects as agreed with the Head of Service. Organisational Development The post holder will use recognised organisational development (OD) techniques to contribute to the development of the clinical and managerial aspects of the team. Participating in training to maintain and development OD skills will be required. Third Sector Liaison The post holder will maintain good working relationships with local and national Third Sector organisations as appropriate in order to identify opportunities for partnership working and development of cost and clinically effective pathways.

Public Engagement The post holder will engage with appropriate statutory and non-statutory groups to ensure that a public voice is always part of service development and evaluation. Clinical Specialism A key element of the role will be the development of care plans with people who have multiple complex problems and support the clinical team to help them manage their condition. Advanced Knowledge in a range of long term health conditions. Where professionally possible the post holder will be qualified and act as an independent prescriber.

Health Coaching The post holder will have a specialist knowledge of one or more condition or approach and will lead the rest of the team in developing interventions using that knowledge. Programme Facilitation The post holder will work closely with primary care colleagues to identify people who are at risk of admission. The post holder will have advanced skills in therapeutic communication and be confident in the use of tools such as motivational interviewing, cognitive behavioural approaches and health coaching. Research The post holder will: - Be actively engaged in research activities.

Require contributions to appropriate national programmes encompassing NISCHR priority areas. Where possible maintain links with university colleagues with Research Interests in long term health conditions and condition management. Possibly recruit participants for national research studies. Be encouraged to be a member of local research governance committees such as R&D Committee or the Research Ethics Committee.

Keep up to date with changes in local research governance frameworks. Maintain an interest in developing research methodologies. Training The post holder will: - Involve teaching professional and non-professional audiences. Train and mentor other professionals in a self-management support role.

Use a variety of media to provide education on health-related topics. Develop e-learning packages for professionals and the public. Deliver webinars to colleagues to keep them up to date on their areas of specialist knowledge. Provide research updates to colleagues relating to their area of particular expertise.

Lead regular training events and contribute to corporate training events such as induction as required. Key Responsibilities The post holder will be expected to: - Ensure the planning and delivery of care in line with national and local priorities and agreed clinical standards the delivery of services to people who have post COVID-19 syndrome. Provide a self-management support service to people with post-COVID-19 syndrome and the wider team as appropriate. Work as a member of the multidisciplinary team and contribute to the development of innovative approaches with this client group.

Undertake an assessment and management of people with complex long-term health conditions. Support colleagues working with people with post-COVID-19 syndrome. Supervise and support colleagues working with more complex people. Supervise and initiate research relevant to long-term health conditions.

Lead and participate in clinical audit projects. Engage in and to disseminate findings from research. Collect research data for ongoing projects in line with current research governance frameworks. Provide individual counselling to individuals with complex long-term health conditions and their family.

Actors are training resource for colleagues by delivering lectures presentations to other professionals, trainees and students. Contribute to the maintenance and development of electronic resources to support the work of the interdisciplinary team.
