Mobile Support Officer

We have an exciting and rewarding fixed term contract for someone experienced in working with elderly and vulnerable customers, who has great communications skills and the ability to support customer in their own home in Crewe and Nantwich area.
As a **Mobile Support Officer**, you will provide support to our customer to enable them to live in the community as Independently as possible with a good quality of life. With experience of working within an older persons’ service and/or working with vulnerable groups you will assess our customers’ needs to ensure they are forwarded to the appropriate agencies for support.
You will undertake regular visits to monitor that the support arranged is sufficient and meets changing needs. The role will also involve installing, maintaining, and removing portable emergency alarms in the customers’ homes across a wide range of tenures.
The role is based out in the local community, so you will be able to work independently and effectively mange your own diary to meet the needs of both customers and the organisation.
**What your duties will be**
- To respond to emergency situations both in your own area and within the rest of the team.
- To recognise and act upon possible abuse of vulnerable customers in accordance with the appropriate policy and reporting procedures.
- To assess and order minor adaptations for our customers tenants.
- Making referrals to Occupational Therapist for major adaptations and equipment
- To programme, install and demonstrate portable emergency alarms in properties for new customers and to remove alarm equipment by request.
- To test the alarm equipment regularly, carry out basic fault finding in alarm and BT equipment and identify and repair or report any problems, technical faults found.
- Keep accurate records in relation to equipment renewals to ensure an efficient renewal programme can be maintained.
- To take payment for installation of Alarm equipment and ensure payments are processed. To provide the Supported Living service to customers within a specified geographical area.
- To undertake scheduled visits at an agreed frequency. Review progress of any action taken at subsequent visits.
- To complete a Support Plan and regularly assess each customer’s support needs. Ensure appropriate action is taken to produce the best outcome for the customer, record on the Support Plan and review the progress of the action plan.
- Complete risk assessments and record any needs identified and action required on the action plan, to help prevent deterioration in health, prevent falls, promote healthy lifestyle and activities, and identify inadequate housing conditions.
- Write up Support Plans and action plans competently.
- Recognise the inability of some customers to self-advocate and take appropriate action, including advocating on their behalf where there is no other alternative.
- Ensure are customer feels confident and comfortable to discuss their support needs.
**What you will need**
- To be computer literate and proficient in using a mobile phone
- Good level of Education to GCSE level or equivalent
- Hold a full driving licence and own vehicle
