Senior Triage Clinician

The role of Senior Triage Clinician is designed to support the triage of new urgent (same day) nursing referrals to the planned care teams and patient self-referrals for urgent unscheduled needs. The post will be supported by the existing triage team and SPA supervisors.
Main duties of the job
Shift patterns are currently Monday to Friday 0800-1600 and 1000-1800 alternate weeks, but the post holder will have the option to support the Unplanned Care team on bank shifts at weekends to support the same activity.
Key relationships include the SPA administrators, Planned Care locality teams, Unplanned Care teams and Palliative Care team.

The Single Point of Access (SPA) Triage Clinician (TC) will join an established team in the SPA for Harrow Integrated Care Services.
The main component of this job is to be the first point of contact for all community nursing referrals, triaging and defining the most appropriate service for such referrals and liaising with, and giving remote advice to, patients, colleagues and referrers.
- To promptly and robustly triage, navigate, signpost, and problem solve the needs of complex patients being referred to CLCH
- To prioritise and arrange appropriate visits in partnership with relevant
- Daily review of waiting lists and escalation of delays or
- Maintenance of clinical competency by working 10-20% of time delivering face to face patient
- The TC will be expected to contribute to the wider adult healthcare agenda, ensuring a flexible, responsive approach that meets local needs within
- Work alongside other senior colleagues in CLCH to ensure referrals are triaged and actioned to the most appropriate

- On a trusted clinician basis, on behalf of a range of services, to robustly triage, prioritise, and signpost verbal and written referrals received by the CLCH SPA, to determine the most appropriate service to meet patient needs based on commissioned
- To communicate the outcome of this triage and signposting process in a professional and supportive way to patients, colleagues, services, and
- To take responsibility for onward referrals to other services, as identified at the point of triage, wherever clinical pathways allow
- To complete triage, prioritisation, and allocation templates in the electronic record system in order that clinical rationale for decisions made is clearly
