An exciting secondment opportunity has arisen for an Integrated Professional Lead to jointly lead our Intermediate Care Allocation Team (ICAT) in East Lancashire.

Working as a service lead alongside a health professional, you will be an integral part of a team who are working in partnership with health and social care across Lancashire, shaping the development and future of the service and how it can best serve Lancashire residents. During the secondment period for this post, you will have a specific focus in your role in relation to social care, compliance with adult social care legislation and the delivery of high quality social care practice and functions.

The multi-agency team provides a single point of access to community intermediate care services, both for people being discharged from hospital or to support people to remain in their own home. The team works with referring professionals to progress their initial assessment and provide the relevant guidance and support on the service that will best meet the person's needs. The team undertake assessments in the community providing advice and guidance plus arrange any ongoing care and support for people being discharged from hospital on a 'Home First' Pathway, and together with the Lancashire County Council hospital discharge team ensure the assessments of people discharged into bed-based placements take place under 'Discharge to Assess' arrangements.

The teams collaborative work includes accessing and linking with intermediate care, reablement and other services to support people to remain as independent as possible, both to support hospital discharge and admission avoidance. The team consists of nurses, social workers, occupational therapists, ICAT co-ordinators and social care support officers. You will be using a strengths-based approaches (Lancashire County Council is also implementing the Three Conversations Practice model across all adult social care teams) to ensure that people receive the right support at the right time that enables them to achieve their optimum level of independence and live their life in a way that they choose.

The ICAT service in East Lancashire fulfils a number of functions including the Single Point of Access (SPOA) for the 2 hour Urgent Community Response and the telemedicine service for all care homes in Pennine Lancs. It is soon to become the SPOA for a wider range of community based health services that sit within East Lancashire Hospitals Trust (ELHT). ICAT is fully aligned to the Integrated Discharge Service; both being hosted by the Community and Intermediate Care Division of ELHT. Joint responsibility for the ICAT service is held by both ELHT and Lancashire County Council.

The role is based on a 7-day working pattern so there will be a need for you to work some weekends, enhanced rates of pay are provided for any weekends or bank Holidays worked.
