Burnley, United Kingdom East Lancashire Hospitals NHS Trust Full time

An exciting opportunity has arisen for a Clinical Team Leader to work with the Out of Hours Team within the East Lancashire District Nursing Service.

The post holder will have the ability to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service. This a 7-day service, including weekends and Bank Holidays

The post holder will work closely with the night team, and the clinical triage hub, to support them clinically and provide leadership in a professional development and safe and effective care. The night team provides a service across the whole of East Lancashire and is currently working towards a more integrated model of out of hours care in conjunction with other key out of hour community services. This is an exciting time to join the team, and we are looking for a leader who can support effective change management and clinical development alongside colleagues, patients and families.

The post holder carries overall responsibility for the assessment, planning, implementation and evaluation of patient care within the district nursing service. They will contribute to the organisation, operational management, and evaluation of the district nursing service at night and for the Clinical Triage Hub to ensure that systems are in place to secure a comprehensive service delivery for the local population. They will ensure collaborative working across divisions, with General Practitioners, partner professionals and agencies in order to secure care closer to home and positive outcomes for patients and their families

They will be responsible for the budget for the Night Team ensuring financial balance and any cost improvement programmes are realised. They will provide the leadership, direction and line management necessary for effective and efficient service delivery and will support the Matron in the delivery of strategic and organisational initiatives and priorities, including infection prevention and control, the promotion of harm free care, and reducing avoidable bed days/admissions, whilst maintaining high standards of clinical practice and professional conduct.

The post holder will lead the Night Nursing Team and the Clinical Triage Hub clinically and operationally.
