Reporting to the Integrated Discharge Team, the post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the discharge management team. The post holder will be expected to work alongside ward staff / therapists as well as external partners to facilitate discharge flow. The post holder will be responsible for supporting the Senior Clinical Professional with the roll out of the OPTICA tool across the acute and community hospital sites to ensure safe and timely patient flow. Using appropriate technologies and equipment the post holder will produce all work outputs to a high standard meeting the organisations requirements for Patient Safety, Information Governance and CQC recommendations in adherence with Trust policies, procedure and guidelines.
