Assistant Practitioner

An exciting opportunity has arisen for a band 4 Assistant Practitioner to work within Pembrokeshire Community Clinics. This includes working within leg ulcer, catheter and ear microsuction clinics as required. The post is part time (30 hours), working four days per week and travel is an integral part of this post.

The ability to travel between sites in a timely manner is essential for this post.

Working within community clinics, the post holder is required to undertake identified allocated duties in relation to the care environment, the direct care of the patient/client as identified by a Registered Nurse and in accordance with Hywel Dda Health Board policies and procedures.

Perform duties and activities within the framework of a plan of care, as set and reviewed by a Registered Nurse at regular intervals.

Assist patients/clients to achieve physical and emotional well-being and comfort.

Ensure that any changes in patients/client’s condition or circumstance are reported immediately to the appropriate person/Registered Nurse/nurse in charge.

Utilise transferable skills within level of competence.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Numerous locations providing mental health and learning disabilities services; Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
