An exciting opportunity has arisen for a care coordinator to join the LCHS Community Parkinson’s Service.
The team specialises in supporting people over the age of 16 years old, registered within the Lincolnshire Integrated Care System (ICS), who have a diagnosis of Parkinson’s or Parkinsonism.
The team consists of Parkinson’s Nurses and a Parkinson’s Specialist Physiotherapist who have the skills and expertise to provide specialised support to people with Parkinson’s, and their families, throughout their journey. If you are passionate about providing a service which will directly impact the quality of life of people with Parkinson’s, then this role is for you. You will work alongside the highly skilled team and liaise closely with other members of the multidisciplinary team including GP’s, Practice Care Coordinators, Therapists and Consultants. We ensure care is personalised to the individual needs of each patient and offer a range of review options including, face to face (home or clinic), telephone or video consultation.
As a valued team member your development will be supported through an induction period, one to one meetings, completion of specific Parkinson’s competencies, clinical supervision and Parkinson’s training either in-service or via external services including the Parkinson’s Excellence Network and Neurology Academy.
The post holder will participate as a member of the multidisciplinary team in the assessment, planning, delivery and evaluation of patients/client’s health and well-being needs to support in management of Parkinson’s Disease, under the supervision of Specialist Nursing and Therapy team.
Part of the role will include help to triage calls into the service, facilitate appropriate referrals and signpost individuals to other healthcare professionals, volunteers, and services as required.
Within this post you will develop skills to virtually monitor our patients on our Parkinson’s virtual ward to assist in hospital avoidance or facilitate earlier discharge from the acute trust.
You will participate in the delivery of complex care as prescribed by the qualified nurse or therapy practitioner. Plan and progress patient interventions with autonomous assessing and planning on a daily basis with indirect supervision of qualified practitioner, reporting and escalating concerns to specialist practitioner where changes are observed in patient conditions.

Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.
To accept delegated assessment responsibility, developing individual plans of care to meet the immediate and ongoing needs of the patients and carers. Prioritise and effectively manage clinical time and other responsibilities. To plan, organise and prioritise the workload of other members of staff, with possibility of some supervisory duties.
Educate patients/carers and demonstrate treatment programmes with the indirect supervision of a qualified practitioner.
Demonstrate working knowledge of other provider’s resources and referral systems to ensure patient needs are met, within scope of practice. Maintain good communication with the multidisciplinary teams within health and social services to ensure the relevant professionals are aware of the patient’s current status and make referrals as appropriate.
