We are looking for a Patient Flow Co-ordinator, in the St Ives and St Neots Neighbourhood Teams on a secondment to cover maternity leave.

The role requires significant understanding of complex community patient pathways, as well as analytical and judgement skills regarding patients’ clinical information and their treatment plans to ensure that Neighbourhood team services are delivered in a timely and efficient manner.

Day to day operational management support will be provided by the Locality Business Support Co-ordinator, with the Area Nurse Manager providing leadership and support regarding daily, weekly and monthly tasks and priorities.

The Patient Flow Co-ordinator is a fundamental role within the Neighbourhood team. The post holder will be responsible for the co-ordination and tracking of complex patients on the Neighbourhood team caseloads across health and social care teams such as acute hospitals, GP surgeries, social services, voluntary organisations and other CPFT services.

The successful applicant will co-ordinate clinical meetings, and liaise with teams, services, wards and GP practices to facilitate the provision of safe, timely, joined up care and services delivered by Neighbourhood teams. The role is key in supporting Neighbourhood teams to prevent hospital admissions and ensure safe hospital discharges.

- Coordinate and manage the patient flow co-ordination of functions for Neighbourhood Teams and associated Therapy and Mental Health Teams. This will include management of patient flow through SystmOne (S1).
- Liaise with patient to gain consent and then ongoing liaison and all professionals involved in the patient’s care as listed above
- Take responsibility for tracking patients who are on their Neighbourhood Team caseload and ensure NT staff are aware of any hospital admissions and their progress through the pathway.
- Close and regular liaison with patient’s GP practices to ensure that practices are up to date with each patient’s current health status, acting as a conduit for two way communication and updates regarding specific patients known to GP practices and community health services including neighbourhood teams, specialist services, urgent

care services and community hospitals
- Liaise with Community Intermediate Care Services and other community based services to support timely hospital discharges including ensuring any follow up appointments have been booked and transportation is arranged
- To build and maintain relationships with GP practices, Community Therapy, Mental Health Teams and Community Specialist Services, Adult Social Care and third sector providers to ensure timely transfer of information between the teams.

- To deal in a professional, helpful and sensitive manner with service users, staff and other agencies by telephone or face to face, taking messages, advising patients about visits, referring other issues as appropriate and answering routine enquiries

This may including taking minutes at MDT meetings, and other patient focused meetings, photocopying, faxing, dealing with other forms as required to support the timely delivery of clinical services to patients.
