An exciting opportunity has arisen for a dynamic, organised and motivated person to join the Community Respiratory team.

The role of the Patient Navigator is a fairly new and evolving post, aimed at improving patient experience, timely flow through services and improved access to our Pulmonary Rehabilitation service and our other Community Respiratory Services.

The role will require regular communication and interaction with the people of Morecambe Bay, so a warm, friendly, respectful and approachable manner, effective listening skills and experience of customer service are important in this role.

The successful applicant will have proven experience and skills in NHS clinical data management (including use of EMIS web, RTT, capacity/demand and Patient experience data), as well as demonstrable competency in IT, secretarial skills and mastery of the Microsoft Office suit.

You will need to be highly self-organised, and have the ability to work on your own within the boundaries of your role, whilst also possessing the ability to work well with others, demonstrating a respectful team-work attitude.

To support the patient journey and pathway development experience of local health and social care care service would be advantageous.
- Tracking and improving the progress of respiratory patients through their pathway of care.
- Working with specialist clinicians to utilise capacity and demand data to improve provision and timely access to care services for patients
- Working with care users to understand and improve patient experience and access
- Provide project assistance to PR leads for the Pulmonary Rehabilitation Service Accreditation Scheme (PRSAS) project, patient education programme and the National Respiratory Audit Programme (NRAP)
- Maintain and update relevant databases and audit systems
- Support the team to develop the use of EPR and IT to improve the patient pathway
- Provide line management support to our services administrative staff, including 121s and appraisals.
- Provide cover of general secretarial and administrative duties for the service as staffing cover requires

The post holder will work with the community respiratory team, specifically providing support to the Clinical Specialists, Service leads and Service manager to ensure timely care for all patients.

Our Multidisciplinary Community Respiratory team provide care for patients with long term lung conditions, in clinics and Pulmonary Rehabilitation groups, delivered in a variety of community venues across Morecambe Bay. Our Community Respiratory Service also provides domiciliary care to patients in their own homes.

Our team is part of the UHMBT Integrated Community Care Group, who provide a wide range of health care services outside of the hospital setting across Morecambe Bay (for example Community Therapy, District Nursing, Rapid Response, Community in-patient units and specialty teams).

UHMBT Hospital services operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
