To address the ever-increasing demand for health care services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time.

The role of the Digital Care Coordinator will be to work as part of the Care-Coordinator team to support patients and healthcare professionals to maximise the potential presented by new digital healthcare technology and the associated new ways of working, including:

- Healthcare Apps
- Wearables and remote monitoring technology
- Data and intelligence

Digital care coordination is a mechanism for delivering 21st Century care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.
- Identify patients who currently do not use digital channels for healthcare access
- Work with patients to improve digital inclusion
- Work with partner organisations to promote digital inclusion
- Identify and support patients who are digitally excluded
- Identify patient cohorts requiring additional medical/social support
- Be an ambassador for Digital Health Care - Supporting patients when engaging with digital health apps and supporting staff with the coordination of digital health.
- Support the rolling out the use of digital health across the practice and PCN.
- Act as a point of contact for digital queries
- Demonstrate, educate, and support colleagues in how to optimally use digital technologies and data and intelligence
- Demonstrate, educate, and support patients in how to best use digital services to support their healthcare needs.
- Co-productive working with colleagues and providers to troubleshoot digital health issues for colleagues and patients
- Take the lead in the deployment of new technologies and education of patients
- Support the education of colleagues in line with implementation plans for digital health
- Provide support to the practice teams for patient with Long-Term Conditions appointments administratively and practically.
- Provide on-site support and guidance to the Patient Services Team around digital health management
- Link as part of the local multi-disciplinary team around the common opportunities from digital health and new ways of working
- Support the practice team to ensure adherence to Information Governance including training, policy development and completion of annual assessment
- Support the organisation with all aspects of GDPR compliance.

**Job Types**: Part-time, Permanent
Part-time hours: 24 per week

**Salary**: £10.50 per hour
