Be able to work autonomously and as part of a wider team Review and triage of referrals Completing comprehensive patient centred assessments Completing risk assessments Sharing of information relating to patient mental health history and involvement with services Support and advice around specialist treatment plans Supporting safe discharge planning Working as part of a multidisciplinary team, across the mental health and acute medical trust Working collaborative with service users and families/carers Referring on to and liaising with other agencies Arrangement of admissions to mental health inpatient services (including MHA assessments) where required Be able to work autonomously and as part of a wider team Providing general advice and liaison to support the care of patients with mental health needs within the acute hospital setting Supporting the education and mental health awareness amongst other healthcare professionals
