**About the Role: Lead Practitioner**
An exciting opportunity has arisen for a Lead Practitioner to support Humankind’s service delivery within Reconnected to Health in HMP Northumberland. You will support the Service Manager to manage the day to day functioning and performance of the service ensuring a positive working environment is maintained and a quality service is delivered. You will have supervisory responsibility for designated staff within Humankind’s Reconnected to Health team. You will also manage a caseload of service users, completing and reviewing high quality and comprehensive assessments, risk assessments and support plans, including liaising with family members, significant others and professionals / agencies as appropriate.
- Providing essential day-to-day guidance to maintain the team's functionality and ensure the delivery of the highest quality service.
- Taking responsibility for your team's well-being, fostering a positive team culture, and supporting staff development to unlock their full potential.
- Spearheading targeted improvements in service delivery, in close partnership with the Service Manager, to ensure our clients receive the best care possible.
- Collaborating with management to maintain a positive work environment, promoting teamwork, and upholding the standards of excellence we are known for.
- Integrating and embedding partnership working into our practices to enhance our ability to support our clients effectively.
- Ensuring strict adherence to health and safety regulations, prioritising the safety of both clients and staff.

Reconnected To Health is led by Spectrum Community Health CIC delivered in partnership with Humankind and Tees, Esk and Wear Valleys NHS Foundation Trust. Reconnected to Health launched on 1st April 2020, and provides quality care, promotes opportunities, and supports the best possible outcomes for individuals, their families and communities across Northeast prisons. The successful post holder will match our ambition and commitment to provide the highest standards of service quality and performance within this partnership.**About You**
To thrive as a Lead Practitioner, we would like you to:

- Be deeply passionate about making a difference, with a commitment to fair chances for all.
- Hold an NVQ Level 3 in Health & Social Care (or equivalent work experience) or be willing to work towards it in post.
- Possess experience in the substance use field or a similar background in mental health.
- Have an understanding and the ability to provide interventions in one-on-one settings, including MI, CBT, ITEP, and PSI's.
