Northumberland & North Tyneside Universal Crisis Team are happy to offer exciting Band 6 Community Practitioner opportunity within the Older Adult pathway.
The service operates as a Universal Crisis Team, offering mental health assessment and intervention to service users of all ages, depending on their needs.
We offer a comprehensive induction programme. We encourage learning and development opportunities.St. George's Park is also the base for our Northumberland Initial Response Team(IRT). The team provides 24-hour urgent mental health care to whoever might need it. The Northumberland IRT was launched at the the end of 2015. It is made up of clinical leads, mental health clinicians and experienced support staff.
Anyone who lives in Northumberland who feels they need urgent mental health care can contact the Initial Response Team. This includes service users, carers and relatives.
People who have not been previously diagnosed with a mental health problem are usually referred to the Initial Response Team via their GP, social care professional or voluntary organisations.
This opportunity is specifically within the Older Adult pathway.There will however be opportunities to work across the other pathways with people of all ages given the ageless nature of the service.
The service operates a 24h shift pattern, 7 days per week. Applicants will therefore be expected to work some unsocial hours including night duty.To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.
In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
To deliver evidenced based Clinical and Psychologically based Interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
To provide compassionate care that is based on empathy, kindness, respect and dignity.
To incorporate a client centred philosophy into nursing practice and client centred collaborative.
