Morpeth, United Kingdom Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust Full time

Exciting Opportunity Alert Join the Northumberland & North Tyneside Universal Crisis Team as a Band 6 Community Practitioner in the Older Adult pathway. The Universal Crisis Team provides mental health assessment and intervention catering to individuals of all ages. Our comprehensive induction program ensures a smooth transition, offering shadowing opportunities and examples of completed documentation to boost your confidence. Get ready to embark on a journey filled with learning, development, and support.

At St. George’s Park, experience the heart of our operations with the Northumberland Initial Response Team (IRT) providing round-the-clock urgent mental health care. Established in 2015, this team, comprising clinical leads, mental health clinicians, and experienced support staff, is dedicated to making a difference. Individuals in Northumberland and North Tyneside seeking urgent mental health care, including service users, carers, and relatives, can rely on the Initial Response Team for support.

This position focuses on the Older Adult pathway, with opportunities to engage across different pathways, reflecting the ageless nature of our services. Operating on a 24-hour shift pattern, 7 days a week, the role may require working unsocial hours, including night shifts.
- Feel confident and competent before you embark on any lone working.
- Complete Holistic assessments, always with a second clinician.
- Review and manage casework of service users with complex needs as part of an MDT.
- Provide specialist clinical advice to service users, carers, staff, and various agencies.
- Develop Recovery and Wellbeing focused Care Plans in collaboration with service users and their families.
- Ensure carer involvement in Care Planning where necessary.
- Deliver evidence-based Clinical and Psychologically based Interventions (opportunities to train in interventions).
- Work collaboratively within the multidisciplinary team to provide high-quality individualised care emphasising safety, recovery, wellbeing, and independence.
- Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning as part of the MDT.
- Provide clinical supervision to staff as directed by Pathway Clinical Lead and Pathway Team Manager.
- Offer compassionate care based on empathy, kindness, respect, and dignity, incorporating a client-centred approach into practice.

We aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future. In return we can offer a dynamic working environment in which to build a career.
