Job summary

An exciting opportunity has arisen for a Band 6 Registered Nurse, to work within the Universal Crisis Team in Northumberland and North Tyneside . This role would be within the Working Age Adult pathway, leading on the Physical Health Pathway. Whilst the physical health aspect of the role will be given protected time, there will also be an expectation that the successful candidate will fulfil all other roles of the Community Practitioner within the crisis team.

The core working hours will be dayshift between Monday-Friday 9-5, there will be the option of condensed hours. Currently the physical health clinics are held on Mondays and Wednesdays therefore the successful candidate will need to be available to cover these days.

The Team forms part of an ageless service working very closely in collaboration with the younger and older persons pathways of the Universal Crisis Team with the principal objective of offering assessment and home treatment to working age adults experiencing a mental health crisis, as an alternative to hospital admission.

This role is based at St Georges Park Hospital in Morpeth although covers the localities of Northumberland and North Tyneside, offering the opportunity to work across areas with beautiful countryside and stunning coastlines. As part of this Team you will be encouraged to grow and develop in your role and to be involved in service development and improvement.

Main duties of the job

The Community Practitionerrole includes delivering home based treatment, completing urgent triage/assessment for those presenting in mental health crisis and shift coordination. The post holder will be required to provide interventions and specialist advice regarding mental health and risk to service users, their families/carers and a wide range of professionals. The successful candidates will have responsibility for a shared caseload working within and supported by a skilled and experienced MDT to deliver safe, effective, responsive and compassionate care to our service will have clinical experience in assessing, formulating and managing risk and complexity within mental health/learning disability services.

The post holder will be expected to lead on the internal physical health clinics and completion of the physical health assessment form for service users. Skills and experience in completing baseline physical health observations, phlebotomy and ECG as per Trust guidance are essential. The successful candidate will work in collaboration with the service user to produce a physical health and wellbeing plan, liaising with team medics, non medical prescribers, pharmacy and GPs as required. The post holder will provide brief interventions in smoking, weight management and any substance/alcohol use where appropriate, they will provide follow up for any referrals/signposting and deliver physical health foundation skills training.

Qualifications

Essential

Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist Proven track record of post registration clinical experience Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
