**Purpose Statement**

The role is designed to be based at The Fyffe Centre in central Lowestoft as part of a wider team focusing on those undertaking, or at risk of undertaking, significant anti-social behaviours including knife crime, county or local drug lines and/or behaviours which jeopardise the safeguarding of others or themselves.

Operating within a small team providing in-reach and out-reach support the post holder will provide high level support to those meeting the relevant criteria and provide specialist advice, guidance and support to their clients in their journey.

The role will require flexible working practices and include taking the responsibility for all client’s immediate requirements when on shift even where this may not be focused on their specialism.

The role will be supported by other team members who will fulfil other specialist level support services as part of their wrap around support which will be case held and monitored by the Intensive Housing Coordinator.

The post holder will also work in a wider community context, providing outreach support and services as well as engaging in wider community engagement through regular events held at The Fyffe Centre focused on inclusivity and proactive as well as reactive elements.

**Main Responsibilities**:

- To work in conjunction with the Intensive Housing Coordinator to identify clients with significant anti-social and criminal behaviours.
- To provide individual and bespoke support, information, signposting and access to appropriate and relevant services for those wishing to improve their presenting factors.
- To be involved in supporting those who are undertaking or risk of undertaking relevant factors to the specialism of the role.
- To liaise with the appropriate individuals to ensure the safety of clients and staff.
- To liaise with external agencies regarding presenting clients and any relevant history or risk factors.
- Support clients to be more positive about their local community and engage in a positive manner.
- To encourage clients to share knowledge and feedback, in order to provide a holistic and efficient service.
- To record and evaluate the impact of interventions using appropriate evaluation tools.
- Ensure that appropriate records and documentation are inputted in a timely manner and stored securely on our CRM system, ensuring concerns are reported to the relevant person(s) in line with organisational governance structures.
- To ensure the service is operational and supportive over multiple locations in accordance with referrals and demand.
- To undertake specific and specialised training in order to support the client group and maintain continued professional development through attending agreed training and conferences.
- To provide interventions that are evidence based and to follow person centred principles. This will involve assessing, planning, implementing and evaluating the outcomes of interventions and maintaining appropriate records.

**Person Specification (Essential/Desirable)**

**Experience**
- Applicants must have previous experience of working within a supported accommodation environment and supporting vulnerable people / families with issues relating to financial hardship, homelessness, mental health and substance misuse (E)
- Applicants must have a detailed knowledge of services and support available to vulnerable people in need and a thorough understanding and commitment to Safeguarding (E)
- Applicants must hold a working knowledge of health and safety (within a supported accommodation environment and within the community) where lone working is applicable (D)
- Applicants must be able to demonstrate that they have experience in working with internal and external stakeholders within a support discipline (E)

**Qualifications**
- A thorough knowledge of housing related support or a care/support environment (E)
- A professional qualification in housing, social work, community work or similar area of work (D)
- Significant lived experience within factors which relate to our customers. This can include knowledge of substance misuse, the welfare system, homelessness, long term unemployment, mental health, financial hardship (E)
