Hours: Full time (37.5 hours) and part time available

Pay: £26,970 to £30,615 per annum (pro rata for part time working)

**Purpose Statement**

Our new Single Point of Access Team (Access Hub) are very pleased to announce that we are looking for forward thinking, experienced professionals to join our progressive and developing team.

You will be providing telephone based assessment and triage of new referrals across the Access services, offering relevant referral, signposting recommendations and completing holistic assessment of needs for all customers.

This newly designed service will work closely with our internal provision as well as our community partners in primary and secondary care services, specialist services and other voluntary sector organisations. You will be involved in further development of collaborative working practices as the Access Hub work continues.

We are looking for confident and motivated practitioners with excellent communication skills who are able to lead on decision making and who can demonstrate the Trust's vision and values in their practice, ensuring that quality care and support is our priority.

**Main Responsibilities**:

- Act as an expert practitioner, demonstrating care and support competencies and a sound knowledge base
- Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols
- Ensure that customers receive high quality care and support, delivered in a timely manner
- Ensure complete and accurate documentation of each and every customer contact
- Ensure that professional standards are maintained and within the guidance of the Access Clinical Governance Policy
- Communicate effectively with customers and families regarding complex information about their care and support
- Taking a Health Coaching approach provide advice and information to customers, carers and their families where appropriate
- Communicate effectively and work collaboratively with staff across all Access services to ensure delivery of a co-ordinated service
- Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers
- Work autonomously making complex decisions with limited supervision within local and national policies, being responsible for how these policies are interpreted
- Contribute to quality improvement within the service through the identification and initiation of changes, which lead to better evidence-based practice and improved standards of care and support
- Participate in care and support audits, identifying areas of best practice and areas for improvement
- Contribute to the setting and monitoring of standards of care and support
- Help develop the services available to the urgent care and support population to reflect identified needs
- Contribute to the development and maintenance of an excellent learning environment
- Adhere to professional registration requirements if you are a registered practitioner
- Participate in defining your own developmental plan to maintain and develop own care and support skills and ensure own professional development

**Person Specification**:
**Essential**
- Experience of autonomous working and being comfortable to manage a level of risk
- Maintain a professional manner at all times
- Adherence to own professional body requirements and work within the scope of professional practice
- Ability to identify when onward referral is required within locally agree pathways
- Excellent knowledge and understanding of the health and social care systems and operating frameworks
- Previous knowledge and experience of working in a care and support environment and with customers who have complex mental health needs
- Effective and competent interpersonal, influencing, communication, observation and reporting skills
- Strong organisational skills and the ability to analyse situations and be able to make the right decisions
- Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances
- Competent IT skills and the ability to navigate around our Zone Standard systems and software packages (such as outlook, databases, MS office and the internet)

- Specialist knowledge acquired through training in a specialised discipline
- Experience of a wide range of approaches to the discharge of complex patients
- Experience of complex support planning and facilitating
- Knowledge of complex customer management
