An exciting opportunity has arisen for a Band 7, Clinical Nurse Specialist within the Persistent Pain Management Service within Central BCUHB. The post will be based in Abergele Hospital, however, the post-holder will be required to run clinics at other sites across the central service, currently including: Rhyl and Deeside.

We are seeking to appoint a highly experienced nurse with Pain Management experience to act as an expert resource whilst facilitating the development of the Persistent Pain Service. The successful applicant will provide expert care and knowledge for patients experiencing Persistent Pain within the Outpatient setting. They will be expected to work autonomously with an individual case-load as well as working collaboratively within a small friendly multidisciplinary team, including Pain Consultants, Psychologists, Physiotherapist and Occupational Therapists. Experience of group working would be beneficial. The post offers a challenging and rewarding opportunity for the post-holder to develop Continued Professional Development with support and training being provided by the multidisciplinary team.

The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.

Contribute and support the management of the Outpatient Persistent Pain Management Service. Manage and organise own clinical workload. Delegate and disseminate duties to other members of the team and throughout the organisation. Provide leadership and support as required.

Contribute to and develop a comprehensive service that requires autonomy, leadership and clinical excellence. Provide expert knowledge and support to patients, families, nursing and medical professions within the Outpatient Persistent Pain setting. In the absence of the lead nurse deputise and take responsibility for the delivery of the Service.

Clinical

Required to

1. Act as a consultative specialist resource for Persistent Pain management and link in with all members of the health care team within the trust and primary health care team.

2. Triage and Receive direct multidisciplinary referrals to the service and individually for patient management from consultants, physiotherapists and General Practitioners.

3. Manage and provide specialist advice on the treatment of persistent pain patients within the Outpatient setting.

4. Contribute to the organisation of the service to facilitate an appropriate level of care and monitoring for the clinical condition of the patient and the technique employed to ensure patient safety (e.g. Interventional procedures / Pain Management Programme).

5. Assist in providing expert clinical information and undertake clinical risks assessment /potential risks, to promote safety and prevent complications/harm to patients.

6. Autonomously lead and manage the care of a patient case-load in the field of Persistent Pain.

7. Independently prescribe and take responsibility for the clinical assessment of the patients and the clinical management required for those patients. Take responsibility for the prescription as well as the appropriateness of the prescription. (DoH 2003). Advise health practitioners on medication management according to evidence based practice.

8. Relate theory to practice at degree level in pain management, ensuring up to date evidence based pain treatment for patients in line with British Pain Society, NICE and other relevant guidelines.

9. Set and promote appropriate standards of care (e.g. identifying the clinical parameters that require monitoring and actions to be taken in the event of adverse effects/side effects from treatments) and specialised pain management treatments for patients (e.g. treatment plans for patients with multiple pathology and complex needs).

10. Assess, implement and evaluate plans of care within specialised area.

11. Contribute to developing extended roles for nurses in the management of Persistent Pain.

12. Use higher levels of clinical expertise and skills in the specialised assessment of patient’s with complex pain problems..

13. The role requires the post holder to demonstrate flexibility, according to the frequent developmental changes of the service.
