**About the role**

You will manage staff who work with individuals to achieve objectives to improve their health and wellbeing, alongside supporting local communities to coordinate and grow what is on offer in their local area.

Working in a creative, strengths based and personalised way you will support local residents who have had contact with Adult Social Care and their informal carers to find practical and sustainable solutions to achieve their objectives.

You will be responsible for managing a team, focusing on promoting what already exists within our local communities, coordinating this information, identifying gaps and opportunities, developing new initiatives and encouraging the local community to take the lead on managing this long term. You will build strong relationships with local partners, understanding the value of residents and promoting an integrated and collaborative approach.

**About you**
- an understanding of Health and Social Care within communities and experience of working with adults with care and support needs in some capacity.
- a passion for working with people, and leading others.
- a creative and enthusiastic approach to supporting individuals build good networks.
- A clear understanding of reporting, risk assessment and personal accountability.
