Willows Home Care is a family run domicillary care provider in the Penwortham area of Preston.Due to rapid growth we are looking for a care-coordinator to join our team. You will work closely with our Registered Manager and work towards the role of Deputy Manager.

You will work within a team to supervise and grow an effective and efficient home care service within a defined geographic region. All our clients are privately funded and the level of care we provide is exceptional. You must genuinely care about providing an outstanding service as our standards are very high.

**Key Responsibilities**
- To ensure the office portrays a professional image of Willows Home Care
- To ensure the provision of high-quality care services to adults living in their own home.
- To ensure all statutory obligations are fully met.

**Specific Responsibilities**
- To participate in On Call duties as part of Rota as required delivering care to clients where staff are sick or on annual leave.

- To liaise with care assistants, GP’s, social workers, and families.
- To review care plans and Risk assessments and discuss with clients/family
- To audit e-MAR on a weekly basis and monitor findings.
- To carry out spot checks on staff and complete the documentation for audit purposes.
- To receive initial enquiries from potential Care workers and carry out pre-screening (telephone)
- To participate in interviewing, selecting and inducting of new Care workers.
- To plan ahead to cover holidays, training and planned absence.
- To record on the system changes to rotas brought about by short notice absence out of hours.
- To maintain accurate records on the system of all communications with Care workers and clients
- To liaise with social workers, clients,families in relation to changes in client circumstances, condition or needs.
- To provide Care workers with accurate and timely information relating to new clients or changes in requirements of existing service users.
- To schedule shifts using our care management softwear system
- To provide clients with information if Care workers are unavoidably running late
- To provide On Call staff with accurate and timely records of schedules.
- To process holiday requests from Care workers, reporting to the Registered Manager and making recommendations regarding approving or declining such requests based upon the needs of the service.
- To maintain records of sickness, holiday and absence of Care workers.
- To identify patterns of sickness/absence amongst Care workers.
- To report monthly to the Registered Manager all episodes of sickness, holiday and absence of Care workers.
- To record on the system and investigate any incidence of late or missed visits and report such incidents immediately to the Registered Manager.
- To record all comments, complaints and compliments and report to the Registered Manager as soon as they are received.
- To cooperate with the Registered Manager during any investigations.
- Updating company policies and procedures with any changes to legislation.
- Such other duties as appropriate to the grade and nature of the position as may be reasonably delegated

**Requirements**:

- **Experience in the care sector preferably domicillary care a minimum of 2 years**:

- **You must have a valid driving licence and access to your own vehicle**:

- **Excellent IT skills word and excel**:

- **Experience using management rostering system such as Access or Nurse Buddy**

**Job Types**: Full-time, Part-time

**Salary**: £22,000.00-£25,000.00 per year
