Case Worker

Job Summary:
We are seeking a dedicated and compassionate Case Worker to join our team. As a Case Worker, you will be responsible for providing support and guidance to individuals and the management of support workers to provide a Service of Care to service users to enable them to lead as independent a lifestyle as possible. This Care Service will involve a programme of personal care and household management that is personalised for each service user in the form of a Care Plan. Duties will therefore include accepting referrals and arranging home visits to undertake assessments to develop a person centred support plan to assist the service user with the meeting their set outcomes and in so doing will at all times observe and respect the service users’ dignity, privacy and independence as far as practical:
RESPONSIBILITIES:
1. Delivering a ‘best practice’ support service, appropriate to the service user's needs.
2. Ensuring the delivery of individual personalised support packages, reflective of different service user’s needs, including culture and diversity issues.
3. Knowledge of ‘best practice’ in both crisis intervention and planned support working.
4. Providing the necessary support required for support workers to complete accurate, legible logs to contribute to the completion of weekly report summaries for our stakeholders.
5. Undertaking referrals and assessments of potential service users, working with them to identify the most appropriate support for the young person/child needs.

7. Ensuring service users have access to relevant external support services.
8. Working with service users to agree and set goals and actions.
9. Supporting the Family support workers to meet clearly defined service user’s outcome targets through key-working, Care plans, risk assessments and other interventions.
10. Ensuring that opportunities for education, training and employment opportunities are integral to the support service. The aim being to enable service users to develop the life skills to live as independently as possible.
11. Maintaining accurate and timely records of all activities including the maintenance of a support planning database.
12. Providing practical support and information to service users when entering the service and throughout their involvement with the service, e.g., assisting service users in the organising of support plan agreements.
13. Working in line with ‘health and safety’ and safe guarding requirements to ensure the overall safety and security of the service users.
14. Provide supervision to support workers as relevant to your case allocation and in line with the service policy and procedures.
15. Promote and encourage a high level of service users’ involvement, consultation and communication.
