**Accountability**:PCN Lead Occupational Therapist, PCN Transformation Lead & Project Manager

**Key Relationships**: Patients, patients' families and support networks, practice staff, PCN colleagues, other health and social care providers, the voluntary/independent sector

**PCN Overview**

Leeds Student Medical Practice and The Light Surgery form our Primary Care Network (LSMP and TLS PCN), which works collaboratively to ensure high quality, equitable, and sustainable care. We work together to represent over 57,000 patients and strive to provide outstanding patient service. Our work remains rooted in the local communities that we serve which is predominately a student population.

**Role overview**

This Care Coordinator role at LSMP and TLS PCN will be involved in a wide range of work to improve the outcomes of patients who need additional support in navigating the health and care system. This will include working directly with a diverse patient list, coordinating with a wider multidisciplinary team to tackle health inequalities, and engaging with patients and colleagues around the benefits of personalised care.

**Primary duties and responsibilities**
- As a Care Coordinator, the postholder will:_

Support the patient journey directly by:

- Developing personalised care and support plans to help manage patients' needs and improve their health outcomes
- Working with patients, their families, and their wider support networks to improve the understanding of the patients' conditions
- Providing coordination and navigation across the health and care system for patients, through answering patient queries and making and managing appointments
- Assisting people to access personal health budgets and benefits where appropriate
- Measuring the impact of your work through patient reporting to identify areas where your role can be developed to improve outcomes

Help people to support themselves through:

- Improving their knowledge, skills, and confidence in engaging with their health and wellbeing, including through use of the patient activation measure
- Enabling them to take up training and employment, and accessing benefits where appropriate
- Assisting people to access self-management education courses, peer support or interventions that support them in their health and wellbeing
- Raising awareness of shared decision making and decision support tools to help enable positive conversations between patients and healthcare services

Work alongside a multi-disciplinary team to:

- Manage a caseload of identified patients who would benefit from care coordination
- Promote the benefits of personalised care to colleagues and improve their understanding of services available to patients
- Identify patients that would benefit from additional support across the system
- Improve any processes that act as barriers to effective personalised care

Proactively work as part of the PCN team to:

- Improve the population health by getting involved in projects around health inequalities within the system
- Increase the number of people attending cancer screening appointments
- Improve the health outcomes of populations experiencing health inequalities within our PCN, working both internally and with external organisations
- Identify and support underrepresented groups such as unpaid carers to access resources and services that will benefit them

**Additional responsibilities**
- As a member of the PCN, the postholder will:_

Build working relationships by:

- Working as a flexible member of the PCN providing support to other team members when necessary
- Encourage supporting staff to collaborate through sharing information and intelligence across different activities

Participate in any training and development implemented by the PCN as part of this employment, including but not limited to:

- Annual individual performance reviews
- Maintaining a record of own personal and/or professional development
- Taking responsibility for own development, learning and performance and demonstrating skill and activities to others who are undertaking similar work
- Keeping abreast of national and local guidance on PCNs

Strive to maintain high quality and governance within the PCN, by:

- Alerting other team members to issues of quality and risk.
- Assessing own performance and taking accountability for own actions, either directly or under supervision.
- Working effectively with individuals in other agencies.
- Effectively managing own time, workload, and resources.
