To support the Trust in maintaining and improving the patients’ journeys from Referral to Treatment by collecting, processing and actively seeking solutions to ensure that each patient is managed throughout their pathway in line with National and local targets.

Main Duties

To take responsibility for the administrative process of the clinical pathway for patients at various stages of their clinical journey in receiving treatment.

To understand current and future requirements for individual patients and take proactive steps to ensure that the pathway is maintained.

To support and liaise with the clinical teams etc in progressing patients journeys within national guidance and improve patient care and experience.

To alert the service providers, through the escalation policy, where personal intervention has been unsuccessful in a timely manner.

To support the Administration Team to manage and track the patients through every stage of the patient pathway on a daily basis, to ensure diagnostic tests and any subsequent treatments are achieved within targets and escalate any potential delays in pathways as protocol which will result in breaches as soon as these are identified.

MAIN DUTIES AND RESPONSIBILITIES

General
1. To take responsibility for the administrative process of the clinical pathway for patients at
various stages of their clinical journey in receiving treatment.
2. To understand current and future requirements for individual patients and take proactive steps
to ensure that the pathway is maintained.
3. To support and liaise with the clinical teams etc in progressing patients journeys within national guidance and improve patient care and experience.
4. To alert the service providers, through the escalation policy, where personal intervention has been unsuccessful in a timely manner to allow corrective action to be achieved.
5. To ensure accurate and timely capture of data to meet the standards of the Trust to support quality improvement of the service.
6. To provide regular feedback to the team leaders and general managers as deemed necessary.
7. To be highly efficient in the use of computer and information systems where data collection across multiple systems is required, recognising the vital requirement for the highest levels of accuracy.
8. To appropriately handle sensitive and confidential information.
9. To support the Administration Team to manage and track the patients through every stage of the patient pathway on a daily basis, to ensure diagnostic tests and any subsequent treatments are achieved within targets and escalate any potential delays in pathways as protocol which will result in breaches as soon as these are identified.

10. Maintain close communication with the Service Mangers updating them on a regular basis on work progress and alerting them to of any problems that may result in a delay in the patient’s treatment.

Organisational
1. To organise own day-to-day workload. Supervision is given by the Surgical operational
management team.
2. To work in accordance with all departmental and Trust policies and procedures.
3. To participate in departmental audit/surveys as required
4. To ensure, in accordance with the Health and Safety at Work Act (1974, to take reasonable care for the safety of himself / herself and other persons who may be affected by his / her omissions at work.
5. To attend mandatory training sessions as determined by the Trust.
6. To comply with the Trust Manual Handling Policy
7. To report all accidents, incident and complaints in accordance with Trust Policy.
8. To support training of new members of staff
9. To undertake any other duties as appropriate to the role.
10. To prioritise and manage a defined workload.
