The role of the Dementia Specialist Practitioner is an important leadership position that will ensure excellent standards of care for all patients living with Dementia at Lancashire Teaching Hospitals NHS Trust (LTHTR). You will have experience of working with people living with Dementia and ;
- be pivotal in progressing the LTHTR Dementia Strategy and guiding the Dementia programme across the Trust.
- will be responsible for ensuring the use of the Forget-Me-Not document and promoting the implementation of reasonable adjustments for patients living with Dementia.
- will inform and progress service developments and projects to meet the aims of the National Dementia Audit, implementing change across the Trust to ensure dementia friendly environments.
- will provide education to our workforce in Dementia, and how best to support patients with behavioural and psychological symptoms of Dementia.
- will support the delivery of person-centred care and promote a positive patient, family and carer experience.
- will have an ability for report writing, audit, presenting findings and evidencing compliance with regulated activity (CQC standards) and high-quality standards (NICE guidance).
- will ensure any restrictive practices for patients are conducted in a safe manner, with guidelines and appropriate legal frameworks considered including the Mental Health Act, Mental Capacity Act and Deprivation of Liberty Safeguards.
- To work alongside the Matron of the team to deliver and drive forward the Dementia Strategy 2021-2024
- To ensure the Trust workforce is upskilled into providing high quality care for patients living with dementia - including review of E-Learning and developing/delivering face to face training.
- To ensure our patients living with dementia have access to activities, cognitive stimulation and reasonable adjustments for a positive patient experience.
- To work alongside families and carers.
- To work alongside MHLT and specialist teams to ensure evidence-based dementia care.
- To ensure legislation such as the mental capacity act, mental health act, Deprivation of Liberties and Best Interest process is followed for our patients living with a dementia.
- To work alongside Safeguarding Practitioners, Mental Health, the Learning Disability Practitioner and MCA/DOLS Lead if required for individual cases.
- Audit, report writing and ability to present key findings.

The mental health, learning disability, dementia and high intensity user service sits within the Safeguarding Team.

The Dementia Specialist Practitioner has a unique opportunity to work across the Trust influencing care for all patients living with dementia through training, role modelling, clinical involvement and by sharing relevant information

The team are passionate and dedicated to safeguarding our vulnerable adults and children, to ensuring our patients voice is heard and to lead on service developments which will make a difference to care

The role of Dementia Specialist Practitioner is an important position that will ensure excellent standards of dementia led care and care for patients who have a cognitive impairment.
- To lead/drive forward the Dementia Strategy alongside the Matron and key partners, including the delivery of the Dementia Strategy Steering Group.
- The post holder will promote activities for cognitive stimulation, positive engagement, and wellbeing as part of the Dementia programme.
- Maintain oversight / an involvement with patients under the care of the Mental Health Liaison Team and act as a conduit between LTHTR and LSCFT MHLT/ mental health for patients living with dementia.
- Ensure high quality of care received by individuals with dementia who access and are admitted to LTHTR acute hospitals via face-to-face contacts or specialist advice.
- Provide support to patients, their carers and relatives.
- Manage the interface between mainstream acute hospital services, primary care services and community older adult dementia / mental health services.
- Facilitate open and easy access to LTHTR for patients with dementia, including the consideration of Reasonable Adjustments.
- To work with MDT members of the Elderly Care Governance Group to support positive patient experience.
- To support the implementation of dementia led evidence-based interventions.
- To understand risk behaviours and vulnerabilities that may result from behavioural and psychological symptoms of dementia

You will act as an expert resource and point of contact for all LTHTR staff and other professionals.
- To provide support and advice regarding care planning for patients
- To consider the difference between dementia, cognitive impairment, and delirium.
- To develop and deliver dementia training.
- To provide support and advice regarding safeguarding issues specifically for patients with dementia, with evidence of safeguarding knowledge and ability to work alongside the Trust safeguarding practitioners.
