Our Care Coordinators play a vital role in our mission by making sure local care operations run smoothly and successfully for everyone.

Main Resposibilities include but are not limited to:

- Creating weekly rotas on People Planner (Experience of Access Software is highly beneficial but training will be provided if you have used other rota systems)
- Managing care needs with clients and their families
- Dealing with care professionals' enquiries and resquests
- Keeping essential records up to date
- Working with the Registered Manager to ensure a safe, effective and efficient service
- A key member of the on-call and back-up team
- Some work in the field with clients is required from time to time

**Salary**: £26,000.00-£30,000.00 per year
