Skimped Hill Health Centre are currently seeking a Health Visitor to join their team for 12 weeks initially. This is a full time role working Monday - Friday 9am - 5pm.

**What you will be responsible for**
- Offer a comprehensive health visiting service in accordance with Berkshire Healthcare Foundation Trust (BHFT) policy and guidelines and ‘The Healthy Child Programme 0-5 years’.
- Assess child and family needs, using the appropriate assessment and risk assessment tools. Ensure that at each contact with a child an assessment of the health and developmental need is conducted.
- Develop and implement outcome focused care plans and evaluate care given, in partnership with clients.
- To provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.

**You will have the following qualification/experience**:

- A minimum of 6 months paid experience as a Health Visitor working for a UK hospital in the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.

**Depending on the Trust, you may require variable additional certification and skills.**
