Externally, the post holder will build upon and forge new links across the faith community, and be pro-active in networking with Social Prescribers and agencies such as End of Life Partnership PURPOSE: To provide spiritual, pastoral and religious care to patients & families, colleagues and visitors within the hospice community, and to support the same in bereavement and at other times of need. Such support to be offered the same regardless of faith or life stance. To work as a flexible and responsive member of the wider multi-disciplinary team and contributing to patient care through thoughtful and appropriate spiritual and/or religious support.

Being a listening ear to colleagues and others, and to those seeking support in bereavement. Providing resources that enable the hospice community to share in wider festivals, Remembrance Services and other national events. Key Tasks and Responsibilities To provide spiritual, pastoral and religious care to patients & families within the In-Care Unit (IPU) and Day Hospice, and to offer the same regardless of faith or life stance. To support and participate in monthly bereavement support group sessions.

To listen to and care pastorally for colleagues (staff and volunteers). To help nurture and grow a team of gifted Spiritual Care and Chaplaincy volunteers. To contribute to the ongoing development of creative, accessible and inclusive resources to enable delivery of high quality religious, spiritual and pastoral care. Whilst the above represent the key responsibilities of this role, they are not all deliverable within a single week of 8 hours duration.

To support patients and families in such a way that meets their spiritual and/or faith needs. The role holder may or may not be able to support all of the needs themselves. If not, they would be required to make an appropriate referral (either in-house or externally) to fulfil the needs. To lead or assist in leading short Christian services and in guided mindfulness sessions during Wellbeing Day hours.

To participate on a rota basis to monthly Bereavement Support Group sessions, to help assess and prepare prospective members referred to this service from internal and external agencies, and to be an advocate for this service internally and externally. To assist preparation and leading of seasonal services of remembrance and similar. To liaise with visiting faith leaders, ensuring good communication as required (respecting confidentiality boundaries) To ensure that spiritual care needs and concerns, and requirements of faith tradition are documented appropriately on the patient records. To respect the wish of patients and families to decline spiritual care and chaplaincy support.

To discuss requests to officiate at funerals, weddings and baptisms with the Family Support Manager, and follow agreed guidelines. To be willing to work occasional evenings and be part of a Sunday rota. To share responsibility for the hospice Chapel, ensuring this is a space, which supports reflection for those of any faith or none. Management To share responsibility to spiritual care and chaplaincy volunteers.

If applicable, to be responsible for compliance with own professional body requirements in terms of code of conduct, professional practice, expertise and knowledge including maintaining continuous professional development. Research and Audit Participate in clinical audit where indicated. Respond to identified needs, developing / modifying practice where required. Quality of Service Provision of a safe environment where patients can freely express themselves without fear of judgement or condemnation.

Ensure the spiritual care and chaplaincy care is pro-active, responsive, inclusive and available to all patients, relatives and carers, and to the wider hospice community. Education To support the induction of staff and volunteers on the team. To be involved in educational programmes arranged for employees, volunteers and other external participants, as appropriate. To take part in the provision of opportunities for in-service training to enable team members to develop, their understanding of spiritual care and chaplaincy across a multi faith/no faith arena.

To help publicise the Spiritual Care and Chaplaincy Team to ensure that patients, families and carers are aware of the support we offer. To represent the Hospice within the wider community, assisting in developing services that meet the needs of all stakeholders. This will include meeting with local Church leaders and leaders of other faith/cultural communities, and contributing to talks and presentations to local and regional faith and community groups.
