An exciting opportunity has arisen within the urological cancers administration team at Charing Cross Hospital’s Urology department. The successful post holder will be primarily providing administrative support to the Urological Cancers Administration teams and thus will be required to work as part of a multi-disciplinary team alongside consultants, registrars, nurses, other pathway coordinators within the urological cancers administration team and operational managers.

**Key areas of responsibility include**:

- Responding effectively to queries from patients and other external parties in line with Trust values, contributing to a positive patient experience.
- Providing administrative support to the relevant clinical teams ensuring the patient's journey is managed efficiently in accordance with referral to treatment targets and agreed timescales.
- Ensuring that letters and other relevant documentation reach appropriate people, including patienst and other healthcare professionals, within agreed timeframes and that all correspondence is appropriately recorded on the Trust's patient administration system.

CLINIC COORDINATION

Implement and maintain an efficient internal system and process to ensure the outpatient access target is met for all new referrals to the Imperial College Healthcare NHS Trust.

Ensure effective communication networks are developed between all organisations that transfer GP referrals to the Urology service.

Work closely with other administrators and clinic staff regarding individual patient queries to ensure targets are met and patients are treated efficiently and equitably.

Monitor and review patient care and/or treatment progress using the Patient Tracking List as well as identifying the next steps in the patient care pathway in line with Trust and local policies.

Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies.

Co-ordinate and provide administrative support to the relevant teams to ensure that all the relevant paperwork and clinical information is available.

Update on all enquiries regarding patients’ care and/or treatment on a pathway from external Trusts and General Practitioners in a timely manner.

Develop an understanding of the milestones of the allocated pathway for patients within a designated group.

Attend relevant meetings and complete actions regarding the care of patients pathway.

Monitor patients on their pathway and proactively find resolutions to improve the speed of treatment by working directly with clinicians and managers.

Action day-to-day issues, ensuring resolution and escalating serious issues to management as appropriate.

Escalate any issues and breaches of the Waiting Times standards to the relevant management in line with agreed escalation procedures.

Ensure the timely and efficient transfer of patients and information between trusts/other organisations involved in the patient pathway, adhering to agreed communication pathways and protocols.

Ensure that all health records are appropriately tracked and securely stored whilst in the department according to Health Records Policy.

PATIENT / CLIENT CARE

To manage distressed, upset or angry patients on the telephone or face to face; to determine the complexity of a situation in order to resolve any problem, and if unable, to refer to the appropriate person.

To be, at all times, courteous, attentive and project a professional image in accordance with Trust standards.

Ensure that requests from patients, their carers and GPs for information are dealt with efficiently and effectively.

PERFORMANCE AND INFORMATION MANAGEMENT

To collate the presentation of information for statistical reports as and when required

To ensure robust processes for accurate recording and capture of patient activity on to Trust IT systems.

Collect data as required in relevant meetings, allowing the Trust to identify and improve treatment in patient cohorts.

COMMUNICATION

Enhance existing good working relationships across the different outpatient/inpatient departments and booking office by establishing communication structures.

To ensure high standard of filing system - both paper and electronic - so that all information is readily available for access by relevant staff.
