- Developing and implementing care plans, ensuring they are person-centred and promote independence
- Coordinating and liaising with relevant professionals and agencies to ensure the best outcomes for service users
- Providing advice, guidance, and support to individuals and their families
- Monitoring and reviewing the quality-of-care provision

**Qualifications and Requirements**:

- NVQ Level 2 in care or a similar relevant qualification.
- Experience in assessing and supporting the needs of vulnerable adults.
- Sound knowledge of relevant legislation and best practices in adult social care.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, colleagues, and external stakeholders.
- Strong organizational and time management abilities, with the capacity to manage a varied workload effectively.
