The Discharge to Assess Team (DTA) is a multi-skilled team consisting of Nurses, Social Workers, Occupational Therapists, Physiotherapists, Community Care Workers and Support Workers. Our purpose is to support a safe and timely hospital discharge of people who are medically fit, and no longer need hospital care, to a place more suitable to the persons needs. Assessments and care provision can then be tailored to support people to regain strengths and skills so that they can live as independently as possible.

We are looking for an experienced clinical leader with evidence of supporting innovation and change to work alongside the adult social care and therapy leads within this valuable service. You will be part of a strong team with a clear ethos of supporting autonomy and independence. You will be part of developing the culture of shared learning, shared skills and positive risk taking. You will provide clinical expertise to the MDT, and you will take a lead on key projects within the team including End of Life. You will be supported to develop your own skills, knowledge and experience within this framework, and you will be encouraged to bring creativity and new ideas to continuously enhance the performance of the team and the experience of the people we support.
