**Job overview**:
Cornwall transformation of its core community mental health services; we are looking for someone to continue to develop and lead the relaunch of our current Single Point of Access service, as it transitions to the Co Produced Your Ways to Mental Health.

This is a refreshing & interesting time to join our adult mental health services in Cornwall, as we are focused on developing into an outstanding service.

We are working towards a complete transformation of services in Cornwall in line with national plans to create integrated and collaborative health and social care systems.

**Main duties of the job**:
The opportunity is to lead on the further development, implementation and embedding of partnership working across Primary Care Networks.

We are looking for someone to inspire, design and implement changes to referral management in Adult Community Mental Health. It means that in this post you can be creative, innovative and encourages you to think outside and around the box in further designing & developing this service.

You will work with our Community Mental Health Teams Operational Leads to provide an outstanding experience and care for the people who use our services.

We will support you to lead on driving forward key national changes give you the confidence to manage and lead your own service.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
