We are looking for enthusiastic, motivated, and proactive ACPs who have the drive and tenacity to develop their professional practice and clinical skills, to join our team and shape and develop continued high level efficient and safe patient centred care in acute medicine.

Post holders will need to possess an MSC in Advanced Practice, core skills in Advanced Life support and an Independent and Non-Medical Prescribing certification.

The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the acute medical team and wider multidisciplinary team to meet the needs of patients, and will play a key role in service delivery and adherence to policy and procedures. They will also be instrumental in guiding Trainee and junior ACPs, junior medical and nursing staff in relation to relevant protocols and procedures.

The post holder will be required to work within all areas of the acute medicine footprint; this includes Medical Decisions Unit (MDU), Primary Assessment & Triage (PAT) Same Day Emergency Care (SDEC) Short Stay Wards undertaking primary and secondary assessments and the management of patients alongside medical colleagues.

They will be instrumental in providing relevant education and training for nurses, junior doctors, other members of the multidisciplinary teams and analysing ongoing training needs. The post holder will required to ensure evidence based care is delivered to patients and will be involved in audit of practice, research and service improvement. The ACP should work within the 4 domains of advanced practice including clinical, educational, research, leadership and management. Protected time will be given each week for CPD and to pursue professional interests in service improvement, education and research.

Clinical
- Co-ordinate the care of the acute medicine patients throughout the care pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
- Work autonomously across professional, organisational and system boundaries to proactively develop partnerships in care to improve health and outcomes of care delivery.
- Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
- Highly developed communication skills enabling maintenance of high quality service within a complex and challenging environment with often, conflicting priorities.
- Assist the clinical and nursing teams, at all levels in the assessment and prioritisation of patients in the acute medicine pathway.
- Use advanced clinical assessment skills to autonomously manage patients with complex needs presenting to acute medicine with an undifferentiated diagnosis to ensure that optimum physical and psychological care needs are met.
- Proactively develop treatment plans for patients requiring urgent intervention including fluid resuscitation, oxygen therapy, antibiotics or other urgent treatment
- Undertake procedures such as cannulation, venepuncture, arterial blood gases to facilitate efficient urgent care in the context of acute illness or sudden deterioration
- Initiate relevant investigations such as X-ray, Ultrasound, ECG and blood tests to enable timely diagnosis and facilitate early decision making.
- Prescribe and review medication, appropriate to patient needs in the context of acute/emergency illness, taking into account any treatment given previously for pre-existing medical
