An exciting opportunity has arisen for an assistant lead for our Virtual ward and IV therapy at home services.

The post holder will support the Virtual ward lead and multidisciplinary team, to continue to grow the services and help develop new pathways to ensure that it supports the wider trust in supporting early supported discharge, admission avoidance and outpatient procedures.

The band 7 assistant lead will care for patients, many of whom have differing and sometimes complex healthcare needs, on a wide range of clinical pathways. They will utilise their clinical expertise and wide range of knowledge to deliver high quality evidence research based care.

Support the Virtual ward lead in helping to develop, grow and deliver an efficient, effective patient centred Virtual ward and IV therapy at home service.

Work with stakeholders both within Sherwood Forest hospitals and providers within the wider ICS of Nottingham and Nottinghamshire, in delivering a collaborative Virtual ward service, which achieves the aspiration of 40 - 50 patients per 100,000 population.
